I won't fault your doctors for defering to the hospital with the NIC in times of crisis, but it should be noted...

...that "medically necessary" can mean almost anything when it comes to a doctor deciding to perform a c-section ("labor not progressing" is pretty much subjective and is the common excuse, but doctors have used stupider excuses). A successful vaginal delivery has a lot to do with patience and a doctor who's not jittery or nervous (never mind that such nervousness can carry over to the patient and hinder progress). Policies - either written or unwritten - among doctor's groups is a large reason as to why c-sections rates in the U.S. are so high (30% in the U.S. versus something like 5% in France!) In many cases, if the patient even sneezes, out comes the scalpel (OK, that's an exaggeration...)
Given your patient base (you mentioned you serve 2 or 3 Indian reservations, right?), that's a good group of doctors you have, there.
