Doctor Who fans, I need help with the last episode (spoils inside)

Why was River upset that the Doctor would find out who she really is? Is it because after she reveals her secret to him in Demon's Run he ends up going to hell and back (emotionally, physically, metaphorically, literally, etc) to rescue her as a baby?

What significance did the cot play? The Doctor said it was his and he slept in it. Was this a lie? River implied it was her crib by asking Amy and Rory to read the Gallifreyan on the side. But if it had the name Melody Pond or River Song on it why was the Doctor surprised that River is Melody? I know the bit of fabric from the one Marine is what gave Melody the name River Song.

A lot passed between River and the Doctor without words (the kissy-kissy face, etc). Was this just the Doctor realizing River knew everything the whole time but never revealed it to the Doctor even when they were playing kissy face?
