Heck yes, tell your sister. It's up to her to talk to the friend's mom, though.

We had a girl in our neighborhood that had a bad relationship with her dad (parents divorced, the mom had kicked her out, living with dad and he wanted to kick her out, etc.). Our neighbor across the street's daughter was friends with her.
I forget all the details (i didn't get involved), but she (the bad girl) ended up getting pregnant. I think our neighbor knew it was a bad decision, but she also supported her / was still her friend. I think it was a good lesson for the neighbor, actually.
This was a few years ago. They (the girl and her baby) have been in and out of the dad's house and the baby-daddy's trailer. It's rather sad.
But neighbor girl is going to school and doing fairly well on her own.
I think it's definitely something you should tell your sister so she can talk with your niece.
