for the life of me I can't figure out how mine is so terrible. one medical charge + one indentity theft charge ( now disputed)

I got 16 other credit types reported - 14 are closed - is that killing me? that they are closed. they were all paid/never late.
so 14 closed/never late
2 open never late
1 macy's card showing up ( hello suppose to be closed with no balance and no available credit wtf?)
