So you started over from the beginning instead of just using new game +?

Make sure you start all of the side missions, and finish as much of them as you can, before finishing the game. Especially Azrael, as some people say you can't finish it post-game. I finished post-game, but not until I had started it on new game +.
Also, you saw the related note about the catch achievement/trophy, right?
