there needs to be some sort of feedback mechanism that actually has a meaningful impact on people's behavior.

For the most part, corrections from your fellow posters are sufficient to change behavior, but not in all cases.
I agree that such a system would need to be democratic, and as such I have no idea how to implement anything of the sort.
