It's so common it has a nickname, "nursemaid's elbow".

It was named that back in the 1800s because the kid's nanny or nurse would grab their forearm or hand and give them a little shake when the kidlet was acting up. Elbow would pop out of joint and the doctor would come over and pop it back into place.
I did it to D when she was 3. She loved to be swung around in circles by the arms. Oops.
