if it didn't mention options/have a combo.of terms in instructions (and in our provider's little note that's included), it'd be a whole lot clearer.

the title is fine.
at least the note from the provider said that receipt of the form didn't necessarily mean action needed to be taken. that's what had me figure it out.
(I knew the share number was correct, I just first thought someone had messed-up how they were reported.)
