If she is not able to make sound decisions you may need to figure out how to get power of attorney so that you can manage her assets to pay for her long-term care.
Does she need 24/7 supervision? If so, you'll need to find a facility to take her in or find a live-in care provider. Neither will be cheap.
It may be that she is eligible for Medicare coverage of (some of) her assisted living care, but she'll probably have to exhaust her assets to qualify. At least that was what had to bne done when my Mom's parents entered a nursing home.
I do not envy you this task, but I hope that it goes as smoothly as possible for you and for her and that she gets the care she needs.
