Gosh I really want more details here, but I see a few possibilities.

1. She's got some issues (and I don't mean that in the wry sense, I mean some actual issues and needs therapy). Hard to pinpoint why I feel that, but I just get the sense.

2. You're not telling us everything. Like about how you might have cheated.

3. She's got classic cold feet and you have to decide if the waiting and wishy-washiness is still worth it, especially if she never comes around.

The above are not necessarily mutually exclusive.
