The health insurance thing is relatively new, but badly needed. The UFC revenue has gone way up with the mainstream tv deal and fighter pay hasn't moved much, although the bonuses for fight/KO/submission of the night have increased 50% over the last few years.
I only called it a co-main event because they did. Other than the title fight (which a lot of people thought would suck, and others couldn't wait to see) it was a pretty weak card. Among my friends who regularly watch the ppvs it was easily the least attended in years. Some of the fights were good and you gotta love Matt Brown. For some reason the heavyweights make more, though the lighter weights usually put on better fights.
Yeah the facebook fights got paid, but the UFC has to have those to fill air time if the televised prelims are short. A lot of fights went to the judges so they didn't need them, but I've seen ppv's where the five fights only lasted 7 rounds, so you ended up seeing everything.
Compared to the regional cards I'm familiar with, the pay for the journeymen fighters is only 50-80% higher in the UFC, although bonuses are much smaller or nonexistant. Bonuses are nice, but there were 24 fighters and only 4 bonuses, so the odds aren't very good...but then if you're trying to make money in mma, you're odds aren't very good at all.
