I hate piling on, but what you have to remember is that not every person "works" in the same way. Some people are great at running on their own.

Others need supervision and guidance, and when they're left to go on their own they just freeze and get confused. Most offices have a number of both types, and you honestly *need* both types.
I'm not excusing her goofing off (at all), but I'll even go so far as to say that if she didn't ask for help, she might not have felt comfortable doing so because she was expected to know what she was doing and freely run with it.
