I've done it lots of times for friends and co-workers.

never for State, just DoD, but I expect it is similar. For DOD, they ask how long you have known the person and in what capacity, how well you know them, etc. They might ask a couple of questions like where they person went to college, as a check of how well you know them. And they ask if the person has a drug or alcohol problem. They may close by asking if you think the person is trustworthy.
