Okay, I need help....Stuck making the leap from awesome on medium guitar to mediocre on hard. What's your secrets?

Especially regarding finger placement.
Responses:
generally (but not always) I find it better to shift my whole hand to the right fret when the fifth button is in use, then back left when it's done.
