Maybe the crutches aren't set to the right height. He shouldn't be putting his body weight on his arm pits.

His hands and arms should do most of the work/lifting and when he resting/not walking he should avoid leaning on them often.

I've had crutches a few times and they're rather uncomfortable to use regularly. Padding will help but he's never going to feel comfortable using them.
