Health insurance is different because if you don't have it someone else pays for it. INEVITABLY.

Because we don't (at least consciously) let people die in the streets.
It's healthcare payment is treated different because it's more like infrastructure. We all get taxed for roads, whether we use that specific road or not. But at some point, you're going to need *a road* if you're a member of society, so we all chip in to make it work for everybody.
