lots of people are outwardly healthy until they're not.

then they go to the ER because they're heart exploded and need expensive emergent care. If they had regular, preventative care perhaps they'd have caught the condition which lead to the heart attack, given some meds and been much healthier (at a cheaper cost) than not.
