And there was always a chance you'd suffer some large-cost episode that you could not cover out of pocket.

however, the hospitals and doctors are still obligated to treat you; and if you couldn't pay...there was still a cost that then would be written off.
How does any business cover sunk costs (for retail this is shoplifting charges)...they raise prices on legitimate consumers.
