If people have the money, they pay it off. If not...they end up getting written off to bad debt. Agencies will try to collect the money...worst case

they sue the patient, get a judgement and then sit and wait to execute on that judgement via lien or garnishment.
In a shit ton of cases, the money is just written off to bad debt and it's gone forever.
