There's generally a cap on the patient's out of pocket and then the rest is covered by insurance up to a pretty large number (now I don't know if

there is a limit annually, but there definitely is not one lifetime). So the answer is usually the insurance company. The overall size of the bill is essentially irrelevant to the payee after deductibles and copays.
