If a doctor says you should get a certain treatment, you might not ask whether it is necessary, or whether a cheaper treatment is available. If a doctor prescribes you a brand-name medication, you might not ask whether there is generic medicine available.
I know what you're saying - that if you're spending your own money you are more likely to ask these sort of questions.
But the doctor will still have more expertise than the patient. "Oh, I think the brand-name gives better results." "The more expensive treatment is safer" "Let's run this test just to be sure" etc...
An insurance company, which has in-house expertise about medical treatment, is more likely to get results from asking these questions than an ordinary patient.
