Require? They cover an annual physical in full - or at least the doc visit portion. In my case it saved me a whole $5 co-pay :)

I don't believe there's any penalty if you don't take advantage of it.

There are dental insurance plans that require you to get at least one check-up/cleaning per year or else you lose benefits the following year. -- nm

True - but if you did go to the doc for something I could see them saying "let's just classify this as your physical."
