Hmm.. well thankfully I haven't seen it go wrong. But a difficult airway (the intubation) can be a problem. However we have fancy toys like an

intubation blade with a camera (which is what I got to use).
Reaction to the drugs.
I think for the most part cruise control is pretty easy, it is the stuff the surgeons do that can screw things up. If they have a bleeder we need to adjust and give pressers to keep the blood pressure up. You have to follow all the changes and give appropriate drugs to keep the homeostasis.
