At one point in the movie, Randy is being sold a few bottles of insulin, along with the other drugs. A few minutes later, you see him injecting the insulin into his ass.

Why would he have wanted/needed insulin? Excess insulin can kill a non-diabetic, so I'm not sure why he'd be taking it. It didn't seem as though he was supposed to be a diabetic in the movie, it was never addressed again.
