"Dear Prime Minister" or "Dear Prime Minister Jerkface" or "Dear Mr. Prime Minister". In the header, it's something like:
The Right Honourable Stephen Harper, P.C., M.P.
Prime Minister of Canada
Ottawa, ON K1A 0A3
(I don't know what the P.C. means)
