Just requested roll-over from my now defunct 401k to a IRA. I'm so sure I screwed up somehow.

I filled out the form, they'll cut a check to SunTrust in care of me. So I think I'll be avoiding all the penalties and such. But I worry about things I don't quite understand such as this.
