What seems to happen is that if there is someone else worthy of elimination, they'll consider prior performance. If not, someone can be eliminated for a single slip-up. There was a guy named Tre in season 3 who looked like he might win the competition. In a single episode with team play, his dish was the worst and the reason the team lost, so he wound up going home. He had two of the last three competitions prior to that, so it was completely out of nowhere.
That's a rare turn of events, but it does show no one is ever a mortal lock.
