The issue is that some theatres won't play NC-17 movies.

And some stores and rental places won't carry them. And some papers won't show ads, nor will some tv or radio stations.
But in answer to your question, you might be surprised if you haven't been to an R movie in a while. Which is understandable, given the lack of them. Idiots drag their kids to these movies all the time.
