I'm terribly unorganized myself, so the only thing I can offer is the simpler the system, the better.

So for example, a cornerstone of my "system" is "if something is still in my email inbox, it's not done. once it's done, it gets foldered. so if my inbox is out of control, i definitely have a lot that i need to do."
I was also having some success for a while running a work to do list on my phone, using Astrid, which is a pretty cool app. I could also see using Catch the same way, though I think Astrid is a lot more clean and geared towards, "hey you, dumbass, get some shit done."
