when i was 21 i had to go in for some surgery (large cyst removed). i was being wheeled into surgery and they left me in the hallway for a second on the gurney. being as i was just laying there, i sat up and picked up my file that they left by my feet.
the very last two lines on my surgery form were
(box to check) - DISCHARGED ALIVE
(box to check) - DIED
i remember thinking "really? if i die they just check a box???"
then they took my chart away from me and told me i shouldn't be looking at that.
