Not for nothing, Tivo, because I do like you as a company, but this was admittedly not the response I expected from our chat this evening:

TFox: I'm having some issues with my new Tivo Premiere where cable channels are gradually having their video and audio fall out of sync when watching live TV. The longer you watch, the more the audio creeps ahead of the picture. If I change the channel, and then change back, it resets and everything is in sync again.
William: I see, this is actually a know issue and unfortunately we have no solution for this yet. However we are looking into the issue trying to get it resolved, for the time being there is a better work around for this issue by just pausing the video it should sync back up.
TFox: Oh. Well that's concerning. Do you have other boxes that aren't defective that this is not an issue for?
William: I say better cause if you are watching live TV and are behind live that will reset the buffer and lose some show as it snaps back to live TV.
William: Not to my knowledge, the older TiVos might not be having this issue.
TFox: Is Tivo planning to notify customers when there is an update to correct this issue?
William: Unfortunately we would not be able to notify but you can contact us periodically to check on status.
(Really? "Yeah, we know that you can't watch live TV correctly" is a known unresolved issue?!)

Ah, the "known issue." I feel for the customer service rep because there's no good answer for that. -- nm

that's.. not ok. I did a quick search to see if there's a sign of how long it's been going on, and I found this - might be worth a shot:
