you should make a change to this, either:

change to a single answer type question because people can select a combination of the first three answers and you would have to clean the data to change those into answer d

or

remove answer d and leave as a multiple answer question because you won't know which combination of answers were meant. Add "Select all that apply" to the question text.

Also, you might want to add gender and age (or just age groups, i.e. 18 to 24, 25 to 34, etc.) if you want any chance of projecting/weighting the responses to a certain group or the population as a whole.
