aside from the fact that you don't know whether you're going to have the original nine-episode order only, or (if you're lucky) picked up for the back 13 to allow you to finish a whole season . . .
things change on a daily, hourly, and minute-by-minute basis for a whole host of reasons. the network gives you notes at every level of the process of creating even a single episode -- outline, script, post -- and that's aside from any logistical issues that arise while you're actually shooting.
honestly, it's a miracle any show ends up being as coherent from week to week as it does
but really, everyone is just making it up as they go along and the only thing that is for certain is that in another 7 days, you have to have something to put on the air
