as I said before, I've seen it both ways for recs/refs. calls/emails are more common than letters for corporate jobs, in my experience.

letters of support for grant proposals have always been non-blind, with or without academic involvement - you have to attach them before submission.
I just did a quick search and found some schools and programs that allow either way - many do want blind, though, of course; and I'd guess transcripts required pretty much always must come from schools, esp. since the high profile scandals.
