Grammar help question please

Imagine that a person (I will use male pronouns here, but gender is not germane) is required to either mail a form or fax a form. Either is fine, but one has to be done. He did neither.

Which wording is correct:

A) He had not mailed nor faxed the form.
B) He had not mailed or faxed the form
C) Either

Responses:
