My theory.

Gomez and Hank are both dead. Somehow they thing Walt's dead too. He uses that as his excuse to run and become Mr. Lambert.
Jesse unfortunately gets kidnapped by Uncle Jack and Co. and forced to make "blue".
What we already know? Walt learns of Jesse's slave imprisonment, comes back, buys a big gun, picks up some ricin, then goes on suicide run to save Jesse, does so, then eats the Ricin himself.
