They didn't meet the criteria and/or the private insurers decided to change the plans. That happened with regularity before the ACA too. This is not

a unique phenomenon. It's just happening in bulk at once because there are new rules. The same is true with increasing rates. It happened before and it's happening (for some, though not others) now.
