My understanding (though I admit the limited nature of my knowledge on this) is that if you were continuing the same plan it was grandfathered.

Nobody has plans that last more than 2 years without being up for renewal (usually 1 year). But the plan had to remain identical. Insurance companies sometimes change the coverages of their plans and that killed some of the grandfathering.
