it was assumed at the time that McGann regenerated into Eccelston. We've learned and seen since that wasn't the case. McGaan regenerated into Hurt who regenerated into Eccelston...

And Hurt counts as a Doctor. He was redeemed at the end of the episode and even though he and tennant won't remember it, Smith will since the event was his. And at the end of the show the all the actors were labled as The Doctor...

exception being Capaldi who wasn't listed...I think more out of respect for Smith being the current Doctor.
