Primal Fear question (huge spoilers)

I only saw it once a long time ago, so I'm unsure about an important detail. Is Ed Norton's character supposed to actually have multiple personality disorder or was it just an act?

The problem I have if it was just an act was that the character who was doing the acting would have never let himself get into the situation that led to the murder in the first place.
