They screwed up the dosing, didn't check on her for over 28 minutes, and didn't have any plan for emergencies.

Major negligence. Which is why you never hear of that happening.

Plus that girl needed 4 root canals and 8 filings. Uh, if she needed that much work at age 3 her parents were at fault as well for negligence.
