I don't think there was anything to it beyond the fact that Sammy Tonin had lost his mind in trying to deal with the pressure of being The Boss.

We watched the season finale of 4 right before the premiere. One of the pertinent lines of dialogue regarding Sammy is that he is paranoid. After a few months in charge, he had clearly lost it.

Dave Foley's line was absolutely correct. "When people start using chainsaws, that's a sign. And it's not a good sign."
