To get it down to why a person in one of those states would not buy ACA coverage...

Someone who makes below a certain percentage of the poverty line would be eligible for Medicare under the expanded coverage. But in states where they opted out, they can't get Medicare *and* they have to pay full price for ACA coverage because they get no subsidies (because the law presumed that they would be eligible for Medicare).
