Germany and US both have 4 points (Ghana tied Germany). Ghana and Portugal both have 1 point. If the US beats Germany, the US would have 7 and Germany 4. Assuming Ghana/Portugal isn't a draw, the winner of that game would also have 4. Then it comes down to goal differential.

Germany currently has a strong lead in differential (Germany +4, Ghana -1, Portugal -4) so it would take a lot to overcome that, but it's in the realm of possible, if unlikely, outcomes.
