A couple hints.

Try to use your DNA points on transmission at first. Make sure to get all of the first level of everything, and the second level of bird, rat, air and water.
Don't add any symptoms at first. If they evolve, devolve them until you get everyone infected. Use a few spore bursts at the beginning, but hold off on using them all until near the end.
When you get a problematic country that just won't infect, spore burst it after everything else has been infected.
