Cost vs Benefit.

short codes are expensive. Also, it appears that $.10/call is as good as I've seen. If there are thousands of patients and this happens a few times a week - it'll become expensive. Cost being scrutinized now in the US medical system to a high degree.
It's extra cost to them for no benefit and as Diva said - actually may hurt their business practices.
A doctor being late is the price we all pay when we're sick, call that day and need to be treated.
Could texting about the office flow happen, sure. But it won't.
