It was, in fact, his fault. He knew he needed to cut ties with Gwen. That's why he pushed the break-up after graduation. Her father's repeated appearance served only to emphasize that point. His continuing relationship with her, after several chances to break it off, is what led to her death.
And really, this point is central to the Spider-Man mythos.
