There was the Scourge invastion which I think was not long before the first expansion

but that only involved nodes outside of the cities that spawned undead.
The new thing (which I gathered from watching Amie play) involves the infection getting into cities and towns. People are complaining about getting killed at the AH and in otherwise "safe" places.
