If you do it once a decade, that's probably sufficient. If they've never been done, you'd be surprised at the crap in them.

We had them cleaned in our last two houses and the amount of drywall dust (which is really abrasive and bad for the blower in your furnace) and construction debris (everything gets push-broomed into the ducts when they "clean" during construction) is mind boggling.
If you have pets you'll see a LOT of fur come out, too.
