Well there are two things going on.

There is a 12DD entry for the amount my employer paid toward my employer sponsored plan. This was there last year too and is just informational.
But what I'm seeing, and maybe this is normal and I've just missed it forever, but this:
My gross income from my final pay stub (not from my W-2, my pay stub)
- 401K contributions
- medical premiums paid by me (and deducted auto from my check)
- dental premiums paid by me (same)
= W-2 income on my W-2
Is that right? Medical and Dental premiums that I pay toward my employer sponsored plan (auto-deducted from my paycheck) are removed from my gross income to determine my W-2 income?
If that's normal (and it surely could be?) then I have never noticed it before.
